Real-World Effectiveness and Treatment Patterns of Tirzepatide in a Large UK Digital Health Cohort

A retrospective cohort study using a UK digital prescribing service found that tirzepatide was associated with substantial weight loss: mean -13.5% at 6 months and -17.4% at 12 months. Most patients achieved clinically meaningful weight loss, more regular ordering was linked to greater loss, and many reported improvements in mood and mobility.

Journal article — Retrospective cohort study. Population: Adults with obesity initiating tirzepatide via a UK digital prescribing service. Sample size: 630 545 eligible patients. Follow-up: 12 months. Interventions: Tirzepatide.

Among 630,545 eligible patients (80.3% female, mean BMI 36.1 kg/m²), mean weight loss in those with recorded weights was -13.5% at 6 months (n = 270,309) and -17.4% at 12 months (n = 58,434). At 6 months, 89.9% achieved at least 5% weight loss and 72.1% achieved at least 10%. More regular ordering was associated with greater weight loss, and clinically meaningful outcomes were often achieved without escalation beyond intermediate doses. Treatment persistence was higher among patients from less deprived areas, who had 26.1% more prescription orders than those from the most deprived areas. Patient-reported improvements included mobility in 46.7%, mood in 45.9%, and reduced alcohol consumption in 26.7%.

This paper provides large real-world evidence on tirzepatide for obesity in a routine digital prescribing pathway, complementing clinical trial efficacy data. It is useful for researchers studying adherence, deprivation, and patient-reported outcomes with GLP-1-based therapies. It does not establish causal efficacy, safety beyond self-reported side effects, or a specific dosing protocol.
