Exosomes released by Cerebrolysin-treated cerebral endothelial cells (Cerebro-Exos) restored the integrity of cultured endothelial cells impaired by fibrin and tissue plasminogen activator, and did so better than exosomes from untreated cells. Their cargo was enriched in metabolic and tight junction proteins, and treatment shifted challenged cells toward reduced inflammation/coagulation proteins and increased tight junction/metabolism proteins. This was an in vitro study and did not test Cerebrolysin in patients.

Stoichiometric analysis showed marked cargo differences between Cerebro-Exos and Naïve-Exos, with Cerebro-Exos enriched in metabolic and tight junction proteins. Cerebro-Exos were superior to Naïve-Exos in restoring CEC integrity impaired by fibrin and tPA. In fibrin- and tPA-challenged CECs, Cerebro-Exos reduced inflammation- and coagulation-related proteins that were augmented by fibrin/tPA and increased tight junction- and metabolism-related proteins that were decreased by fibrin/tPA. No quantitative values are reported in the abstract.
Researchers studying Cerebrolysin would care because this paper identifies a potential paracrine mechanism: exosomes from Cerebrolysin-treated endothelial cells carry protective cargo and can reverse fibrin/tPA-induced barrier dysfunction in cultured cerebral endothelial cells. It does not establish that this exosome pathway operates in vivo or that Cerebrolysin has clinical benefit through this mechanism.
